Our NPP is provided separately and governs all PHI exchanged in the context of a patient-provider relationship.

The Site does not collect PHI through its general informational pages. If you submit an inquiry or appointment request through the Site, any health-related information submitted will be treated in accordance with applicable HIPAA regulations and our NPP.

WAIVER OF JURY TRIAL: TO THE EXTENT PERMITTED BY APPLICABLE LAW, EACH PARTY IRREVOCABLY AND UNCONDITIONALLY WAIVES ANY RIGHT TO A TRIAL BY JURY IN ANY ACTION, PROCEEDING, OR COUNTERCLAIM ARISING OUT OF OR RELATING TO THESE TERMS OR THE USE OF THE SITE.

Any cause of action you may have arising out of or relating to these Terms must be commenced within one (1) year after the cause of action accrues, otherwise such cause of action is permanently barred.
